Anxiety disorders can manifest as fear, restlessness, an inability to concentrate on work or school tasks, and difficulty in falling asleep at night.聽 In some cases, anxiety can arise out of the blue as in a panic attack, when sudden spikes of intense anxiety make the sufferer think they are having a heart attack, 鈥榞oing mad鈥, or even dying.聽 In other cases, it is triggered by specific situations, such as being on a bus or at a social gathering, and symptoms such as sweating, gastrointestinal discomfort, dizziness, and chest pains may ensue.聽

Anxiety disorders are some of the most common mental health problems and their annual cost in the United States is estimated to be $42.3 billion.聽 In the European Union, they affect over 60 million people in a given year.聽

Despite anxiety disorders being common and costly, few studies have looked at what makes some people have anxiety when going through tough times, while others facing the exact same situations are able to maintain good mental health.聽 National Institute for Health Research (NIHR)-funded researchers from the Cambridge Institute of Public Health used data from over 10,000 British women who had responded to a structured health and lifestyle questionnaire.聽 探花直播questionnaire included a measure of Sense of Coherence, which is a personality disposition.聽

Women living in deprivation but who reported the following traits were less likely to have anxiety: believing they were in control of their lives, believing their lives made sense, and having a purpose and meaning in life.聽 Women living in deprivation but without these desirable traits had high levels of anxiety.聽 In fact, women in deprived communities without these traits were almost twice as likely to have anxiety as women living in more affluent communities.聽

鈥淭his study sheds light on inner strengths or qualities that we may have which can protect us from anxiety when we鈥檙e exposed to hardships, such as living in deprivation,鈥 says first author and PhD candidate Olivia Remes. 鈥淔ostering such strengths or traits may be useful for people who do not respond well to medication or other therapies for anxiety, and further research would be needed on this.鈥

探花直播researchers say that living in deprivation can lead to a sense of meaninglessness among individuals, and can give rise to poor mental health and suicide.聽 In deprived communities, people are more fearful of their neighbours, assaults are more likely to happen, and it is difficult to form close relationships with others.聽 探花直播total number of people living in deprivation worldwide is large; as such, the results of this study are particularly important.聽聽聽

鈥淭his study takes a different approach to mental health,鈥 continues Professor Carol Brayne, Director of the Cambridge Institute of Public Health.聽 鈥淯p until now, most studies have looked at what makes someone prone to disease, and the risk factors for ill health.聽 But we have taken a different approach.聽 Instead of looking at risk factors for disease, we are looking at traits or strengths that we have within us that can help us maintain good mental health and overcome adversity.

鈥 探花直播study could help researchers develop new ways to approach how women can be helped to overcome anxiety, and also highlights the key role of context in our mental health.鈥
She has been looking at who is most affected by anxiety, some of the factors that can give rise to it, and the impact that untreated illness can have on society.聽

鈥淎nxiety is not only very costly for society in terms of high health service use, work absenteeism and decreased work productivity, but it can cause much suffering to those affected,鈥 she says.

To carry out her research, Olivia uses data from the European Prospective Investigation of Cancer in Norfolk, one of the largest cohort studies looking at chronic diseases, mental health, and the way people live their lives.

Olivia is keenly aware of the importance of sharing her research with other academics, policy-makers and the public, often through the media. In the European Union, over 60 million people are affected by anxiety disorders in a given year.

There have been few studies to date that assess the factors or characteristics that are linked to anxiety disorders, and even fewer looking at the impact of places where people live in relation to anxiety. However, previous studies have linked living in areas of high deprivation or poverty with significantly increased risks for serious medical conditions and a shorter life expectancy.

To examine whether living in poor areas is related to anxiety disorders, researchers from the Cambridge Institute of Public Health studied health and lifestyle questionnaires completed by some 21,000 people in and around Norwich, east England, between 1993-2000. 探花直播participants had been recruited as part of the EPIC-Norfolk study, set up to look at the connection between diet, lifestyle factors and cancer.

探花直播results of the study are published today in the journal BMJ Open.

One in 40 women (2.5%) and one in 55 men (1.8%) were found to have generalised anxiety disorder. Women living in the most deprived areas were over 60% more likely to have anxiety than those living in areas that were not deprived. This association between deprivation and generalised anxiety disorder was not apparent in men.

Although the researchers acknowledge that it is difficult to confirm that living in deprivation causes an increased risk of anxiety in women, they believe this is what their analysis points towards.

鈥淎nxiety disorders can be very disabling, affecting people鈥檚 life, work and relationships, and increasing the risk of depression, substance misuse and serious medical conditions,鈥 says first author Olivia Remes, PhD candidate at the Department of Public Health and Primary Care. 鈥淲e see from our study that women who live in deprived areas not only have to cope with the effects of living in poverty, but are also much more susceptible to anxiety than their peers. In real terms, given the number of people living in poverty worldwide, this puts many millions of women at increased risk of anxiety.鈥

探花直播team speculate why this may be the case. Women are more embedded in their communities than men 鈥 tending to stay at home more and do more of the domestic duties 鈥 and so the stress and strain of living in impoverished communities seems to affect them more, they argue. Also, women are increasingly taking on multiple roles in society today: income-earner, child-bearer, care-taker 鈥 all of which adds to their burden. However, while men may be less susceptible to anxiety, their stress can lead to other negative coping behaviours such as alcohol and substance abuse.

Professor Carol Brayne from the Cambridge Institute of Public Health, explains: 鈥淎nxiety disorders affect a substantial number of people and can lead to poor health outcomes and risk of suicide.聽 Now we know that women are particularly affected by deprivation, while men less so. This is intriguing and further research is needed on this, particularly in the most deprived regions.鈥

鈥淥ur findings show that mental health policy needs to take communities or the places where people live into account: investing in a local area will not benefit all parts of its population in the same way,鈥 says Dr Louise Lafortune, Senior Research Associate at the Cambridge Institute of Public Health. 鈥淚t鈥檚 evident from our study that we need to take into account gender when determining what action to take.聽 This is particularly important at a time of scarce economic and health-related resources.鈥 聽

Women living in the most deprived areas are over 60% more likely to have anxiety as women living in richer areas.聽 However, whether men lived in poorer or richer areas made very little difference to their anxiety levels, according to new research from the 探花直播 of Cambridge.

Women who live in deprived areas not only have to cope with the effects of living in poverty, but are also much more susceptible to anxiety than their peers. In the European Union, over 60 million people are affected by anxiety disorders in a given year.

There have been many studies looking at the number of people affected by anxiety disorders and the groups that are at highest risk, and in an attempt to synthesise the various studies, National Institute for Health Research (NIHR)-funded researchers from the 探花直播 of Cambridge鈥檚 Institute of Public Health carried out a global review of systematic reviews. Out of over 1,200 reviews, the researchers identified 48 reviews that matched their criteria for inclusion.

Between 1990 and 2010, the overall proportion of people affected remained largely unchanged, with around four out of every 100 experiencing anxiety. 探花直播highest proportion of people with anxiety is in North America, where almost eight out of every 100 people are affected; the proportion is lowest in East Asia, where less than three in 100 people have this mental health problem.

Women are almost twice as likely to be affected as men, and young individuals 鈥 both male and female 鈥 under 35 years of age are disproportionately affected.

探花直播researchers also found that people with other health conditions are often far more likely to also experience anxiety disorders. For example, around one in ten adults (10.9%) with cardiovascular disease and living in Western countries are affected by generalised anxiety disorder, with women showing higher anxiety levels than men. People living with multiple sclerosis are most affected 鈥 as many as one in three patients (32%) also have an anxiety disorder.

According to first author Olivia Remes from the Department of Public Health and Primary Care at the 探花直播 of Cambridge: 鈥淎nxiety disorders can make life extremely difficult for some people and it is important for our health services to understand how common they are and which groups of people are at greatest risk.

鈥淏y collecting all these data together, we see that these disorders are common across all groups, but women and young people are disproportionately affected. Also, people who have a chronic health condition are at a particular risk, adding a double burden on their lives.鈥

Obsessive compulsive disorder (OCD) 鈥 which is an anxiety disorder characterized by obsessions and compulsions 鈥 was found to be a problem in pregnant women and in the period immediately after birth. In the general population, only one in a hundred people are affected by OCD, but the proportion with the disorder was double in pregnant women and slightly higher in post-partum women.

However, the analysis also showed that data on some populations was lacking or of poor quality. This was particularly true for marginalised communities, such as indigenous cultures in North America, Australia and New Zealand, and drug users, street youth and sex workers. Anxiety disorders also represent an important issue among people identifying as lesbian, gay, and bisexual; however, there are not enough studies in these populations, and those that have looked at it are of variable quality.

Dr Louise Lafortune, Senior Research Associate at the Cambridge Institute of Public Health, explains: 鈥淎nxiety disorders affect a lot of people and can lead to impairment, disability, and risk of suicide. Although many groups have examined this important topic, significant gaps in research remain.鈥

Professor Carol Brayne, Director of the Cambridge Institute of Public Health, adds: 鈥淓ven with a reasonably large number of studies of anxiety disorder, data about marginalised groups is hard to find, and these are people who are likely to be at an even greater risk than the general population. We hope that, by identifying these gaps, future research can be directed towards these groups and include greater understanding of how such evidence can help reduce individual and population burdens.鈥

Reference
Remes, O et al. A systematic review of reviews on the prevalence of anxiety disorders in adult populations. Brain and Behavior; 6 June 2016; DOI: 10.1002/brb3.497

Women are almost twice as likely to experience anxiety as men, according to a review of existing scientific literature, led by the 探花直播 of Cambridge. 探花直播study also found that people from Western Europe and North America are more likely to suffer from anxiety than people from other cultures.

Anxiety disorders can make life extremely difficult for some people and it is important for our health services to understand how common they are and which groups of people are at greatest risk

Olivia Remes

Flavia

Anxiety

探花直播text in this work is licensed under a Creative Commons Attribution 4.0 International License.
